By signing this application, I understand and agree to the following:
1. Class fees, registration, and waiver are due 2 weeks prior to class date. Only your
payment holds your reservation.
2. CANCELLATION POLICY: I understand that if my class is canceled by A+ , my fee will be
applied toward the same class on the either of the next two scheduled dates. If I cancel
within 24 business hours prior to the class start date, 1⁄2 my fee is refundable. If I attend
class on the first day and withdraw my fee is NON-REFUNDABLE. If I fail to appear for class,
my fee is NON-REFUNDABLE.
3. A+ Security & Safety Training, Inc. depends upon the careful control of firearms by each
student; therefore, I understand my instruction may be terminated at any time during the
class if the staff deems my cooperation and behavior unsatisfactory. There will be NO
REFUND.
4. Instruction given by A+ Security & Safety Training is for the sole purpose of instruction
for the students attending the class ONLY. We assume no liability for any student attending
a class who uses any information obtained in the class to instruct others.
5. I will abide meticulously by all safety procedures of A+ Security & Safety Training
6. The undersigned shall possess and produce to A+ Security & Safety Training a valid FOID
card at the time of and before any class participation (IL residents only).
7. The undersigned hereby agrees to all terms and conditions set forth in Appendix
“A” A copy of which is attached hereto and incorporated herein.
